The Home Finder Program is funded by the County of San Diego Behavioral Health Services (BHS). Home Finder serves clients who are homeless and are experiencing serious mental illness and are connected to designated outpatient County of San Diego B.H.S clinics. Home Finder is responsible for the provision of housing identification, housing navigation, supportive services and trainings.

Home Finder has Housing Navigators who coordinate, broker, schedule appropriate client services with external medical, dental, and housing to meet client’s needs. Our dedicated Housing Locator is responsible for housing identification including market rate, affordable housing, shared housing, and permanent supportive housing. The Housing Locator and Community Engagement Coordinator attend regional landlord meetings and develop relationships with community landlords. Home Finder staff meets with clients at the designated ( Areta Crowell & North Central Mental Health) clinics Mon-Thursday 9:00am-12:00pm.

Program Criteria:

  • 18 years or older
  • Being treated at identified outpatient County of San Diego clinics
  • Homeless or unstably housed
